New rehabilitation facility for elderly patients

SINGAPORE - In a first-of-its-kind model, Peacehaven Nursing Home will be offering 32 beds to elderly patients who need rehabilitation before going home.

Announced by Health Minister Gan Kim Yong and in partnership with Changi General Hospital, the initiative aims to provide lower intensity rehab of about three to six months for frail elderly recently discharged from hospital.

The Straits Times reported that the facility will be manned by a team of therapists, nurses and social workers.

This to address the gap between hospitals and homes, and allows them to regain maximum mobility before going home.

This in turn improves the chances of them being able to live independently, Mr Gan said.
